    Video streaming on game consoles is on the rise as people increasingly spending time watching services like Netflix, Hulu, MLB Network and ESPN3 on the devices. According to new Nielsen data based on an October survey, streaming on Xbox 360 now accounts for 14% of time spent, up from 10% last year, 15% on PlayStation 3 (up from 9%), and a full third of time on the Nintendo Wii (up from 20%).     An elevator accident at 285 Madison Ave, home to iconic ad agency Y&R, (formerly Young & Rubicam) has left one person dead and several injured, according to local New York reports. The accident occurred at about 10:30 a.m. Wednesday morning, when an elevator began moving between floors with its doors still open. The victims have not been identified and it was unclear if they worked for the agency or sibling firms also housed at the building. Unrelated companies are also tenants in the building.     Mobile ad network Mojiva has named Microsoft veteran Frank O’Donnell as its new chief technology officer. In connection with the move, current CTO and Mojiva co-founder Krish Arvapally has been promoted to chief innovation officer.
